Any entity that falls within this definition, must register with the Wyoming State Board of Pharmacy as a Third-Party Logistics Provider.
Where operations are conducted at more than one location by a single third-party logistics provider, each location shall be licensed by the Wyoming State Board of Pharmacy.
This application requires a Designated Representative. A person can serve as the Designated Representative for only one location at a time. Any time there is a change in the Designated Representative, a new application must be submitted within 30 days.
3PL renewals are now open. Renewal notices are emailed to the email address on file with the Board. Licensees are encouraged to renew online. Email bop@wyo.gov if you do not remember your login credentials. Renewal forms and renewal instructions are also available below. If you choose to submit your renewal form and payment to the Board office, please note:
We do not verify the receipt of renewal applications due to heavy volume, and
We cannot accept renewal applications that are submitted via fax or email.
New applications must be mailed.
Faxed or emailed applications are not accepted.

Change of Ownership:
The Wyoming State Board of Pharmacy does not respond to hypothetical questions and evaluates changes of ownership for 3PLs on a case-by-case basis. In addition to a detailed description of the proposed transaction, which may include an org chart showing pre and post-transaction changes, the Board needs as much information about the proposed transaction as possible to conduct a review.
Change of Name or Address of a 3PL:
Submit the change in writing to the Board of Pharmacy, accompanied by copies of the following documents:
Copy of resident state license reflecting the change
Copy of DEA certificate reflecting the change (if applicable)
Copy of inspection report (for change of location)
