Third-Party Logistics Providers (3PL) Applications

A Third-Party Logistics Provider is an entity that provides or coordinates warehousing, distribution, or other logistics services of a product in interstate commerce on behalf of a manufacturer, wholesale distributor, or dispenser of a product but does not take ownership of the product, nor have responsibility to direct the sale or disposition of the product.

Any entity that falls within this definition, must register with the Wyoming State Board of Pharmacy as a Third-Party Logistics Provider. 

Where operations are conducted at more than one location by a single third-party logistics provider, each location shall be licensed by the Wyoming State Board of Pharmacy.

This application requires a Designated Representative. A person can serve as the Designated Representative for only one location at a time. Any time there is a change in the Designated Representative, a new application must be submitted within 30 days. 

New applications must be mailed.
Faxed or emailed applications are not accepted.

Change of Ownership:
The Wyoming State Board of Pharmacy does not respond to hypothetical questions and evaluates changes of ownership for 3PLs on a case-by-case basis. In addition to a detailed description of the proposed transaction, which may include an org chart showing pre and post-transaction changes, the Board needs as much information about the proposed transaction as possible to conduct a review.
